Age Safely. Live Independently. Thrive at Home.
CAPABLE (Community Aging in Place, Advancing Better Living for Elders) is a person-directed, home-based program that helps older adults improve safety, independence, and quality of life through practical solutions tailored to their goals.
How does it work?
CAPABLE is a four-to-five-month program that brings together an occupational therapist, registered nurse, and handy worker to help older adults remain safe and independent in their homes. Unlike traditional healthcare services that focus primarily on medical conditions, CAPABLE focuses on the everyday activities that matter most to each participant.
Participants identify their own goals, such as:
- Bathing safely
- Climbing stairs
- Preparing meals
- Managing medications
- Walking outside confidently
- Staying active in their community
The care team works alongside the participant to develop practical solutions that improve function, safety, and independence.
What are the benefits?
Improved Independence –Â Participants gain skills and confidence to perform daily activities and remain in their homes longer.
Safer – Home modifications and practical solutions help reduce hazards and improve mobility.
Improved Health – Research shows CAPABLE can improve strength, balance, medication management, nutrition, and emotional well-being.
Reduce Hospitalizations – Studies have shown reductions in hospital stays and nursing home days for participants.Â
Greater Confidence – The participant-directed approach helps build self-efficacy and problem-solving skills that continue after the program ends.Â
CAPABLE is unique because participants set the goals that matter most to them. Success is defined by the individual’s priorities, whether that means safely taking a bath, attending church, preparing meals, or remaining active in the community. The care team helps remove barriers and create solutions that make those goals achievable.
