Program Spotlight: Community Transition Care

MAC Inc. is proud to share the outstanding outcomes of the Community Transition Care (CTC) Program, which supports individuals recently hospitalized in safely transitioning back to their homes and communities, helping to prevent unnecessary nursing home placements and hospital readmissions.

Between March 2023 and June 2025, data was collected on 100 participants, with analysis conducted by the University of Maryland, Baltimore. The results speak volumes:

• 💰 2081% Return on Investment – For every $1 spent on services, $20.81 was saved in hospital-related costs.
• 📉 36% Reduction in Hospital Visits – Visits dropped from 228 to 146 post-enrollment.
• 🏥 59% Decrease in Hospital Charges – Costs fell from $2.19 million to $905,130.
• 🏡 134.8 Days in the Community – On average, participants remained in community settings for over four months post-discharge.

The program served a diverse population of older adults and adults with disabilities, nearly half of whom lived alone. With 97.8% of referrals coming directly from hospitals, the program also demonstrated strong clinical partnerships.

Thanks to its success, MAC Inc. is continuing the CTC Program beyond the original grant period, ensuring that more individuals can benefit from this life-changing support.
