Maryland Access Point (MAP)
Information & Assistance
MAP promotes independence, personal choice, and streamlined access to private and public long-term support services. MAP aims to help you stay active, independent, in the community as long as possible. We serve older adults, individuals with disabilities, and the people who support them.
MAP specialists support caregivers, health care providers, and other aging professionals that work with individuals with long-term care needs.
Eligibility:
- The MAP program is available to everyone at no cost, regardless of income.
How can MAP help?
It can be overwhelming to try to understand your options for long-term support services. MAP is here to help. We are a one-stop-shop where you can work with a certified Options Counselor to:
LEARN about long-term support services, such as assistive technology, home-delivered meals, home modifications, home health aides, chore services, adult medical day care, assisted living facilities, and more.
KNOW your options, plan wisely, and take action before a crisis happens.
IDENTIFY long-term support services that meet your goal to remain in your home or community.
APPLY for available public and private programs and services to support you. Please note: income guidelines may apply for these programs.
ACCESS services and programs that support you or your loved one in the home or community.
NAVIGATE through the complex public long-term care system.
